Bonnie Judd, FNP, APN

Bonnie began her nursing career at St Thomas Hospital.  She worked in cardiac telemetry and then in the cardiac critical care unit from 1992-1997.  Miss Judd then worked 2 years in the home health industry before moving on to Three Rivers Hospital, where she worked as the Skilled Unit Nursing Manager, then as Chief Nursing Officer.  In 2002, Bonnie entered the Masters of Nursing program at the University Of Alabama-Birmingham, concentrating of Family Care.  Bonnie is a Board Certified Advanced Practice Nurse, is Certified by the American Nurses Credentialing Center, and the American Academy of Nurse Practitioners.  Bonnie also serves as a Nurse Practitioner student preceptor for Vanderbilt School of Nursing.  She works in collaboration with Dr. George Mathai, who serves as her preceptor physician.



Kathy Crowell, Assistant to FNP

Kathy has over 10 years of experience in the health care business.  She has worked with patients of all ages from pediatrics to geriatrics.  She is a life long resident of Humphreys County. Kathy handles patient injections, blood draws, and specialty scheduling she is also certified in CPR for healthcare providers

Sandi Rice, Business Manager  

Sandi began her management career in 1989, where she managed several restaurants and then moved on to retail management.  She traveled extensively leading crews of store management personnel in transitioning bought out retail establishments and training their staffs.  At Memorial Clinic, Sandi handles the business financials as well as front office support and employee management.  

Alex Judd

Officially the daughter of Bonnie Judd, Alex works part-time at the clinic. She assists with patient care as well as front office support.  Alex has a special interest in the medical field, she wishes to attend medical school, and study to be a Cardiothoracic Surgeon.

Jenny Hickman

Jenny began as part time staff member while in high school.  After graduation she has stayed on while attending community college.  Jenny has an interest in nursing and has been accepted to Aquinas Nursing Program.  Jenny assists in the care of patients as well as front office support.

Dona Rice

Dona began her career as a homemaker.  In 1985 she began work part-time as a travel agent in downtown Waverly, where she eventually became office manager, until her retirement in 1998. Dona enjoyed a few years of retirement when in 2005 she agreed to help her daughter, Bonnie Judd, open Memorial Clinic.  Originally Dona was to help get things off the ground for a few weeks, but instead has become a regular face assisting with patient care and front office support.

Dyan Farmer

Dyan is a member of Waverly Methodist Church, and is involved in numerous activities as well as holding several positions within the church.  She is also chairman of the Humphreys County Election Commission.  In her spare time she enjoys working in her greenhouse, sewing, knitting, gardening, and reading.  She is married and has two daughters.


She works part-time at the clinic, assisting with patient registration, front office support, and patient referral scheduling.
